Due to the nature of the services that we provide, under NO circumstances are refunds allowed for any services or products purchased at APlus Lifestyle Wellness & Med Spa, PLLC.
BOOKING AND OFFICE POLICY
Intake Forms and Appointment Confirmation:
Intake forms will be sent via email once your appointment is confirmed. Please remember to check your spam folder.
To ensure a smooth appointment, please complete the intake forms prior to your scheduled appointment. Failure to do so may result in rescheduling.
A $50 consultation fee is applicable.
If you decide to proceed with a service on the same day as the consultation, the $50 fee will be credited towards your chosen service.
Card on File:
All clients are required to have a valid card for payment on file.
No Call/No Show Policy:
Failure to notify us of a cancellation will result in a $50 fee.
Late Arrival and Cancellation:
If you are more than 10 minutes late for your appointment, it will be canceled and a late fee of $35 will be charged to your card on file.
Appointments must be cancelled within 24 hours of the appointment date and time to avoid cancellation fees.
